Potential applicants must meet the following Access Principles:
- The individual is currently living in Scotland or is planning to move to Scotland following an out-of-area placement or hospital discharge.
- The individual has a primary diagnosis of Learning Disability.
- The individual is categorised as Red on the Dynamic Support Register.
- The application supports one of the following outcomes:
- discharge from hospital
- return from an out-of-area placement
- to remain in their own home where their current support is at risk of breaking down
All applications should be supported by a current comprehensive and costed Personal Support Plan and / or Discharge Plan. ILF Scotland may be able to assist in the development of such a plan if requested.
Applications to the Coming Home Fund should be submitted by local authorities on behalf of potential recipients.
