- the individual has a primary diagnosis of Learning Disability.
- the individual is categorised as Red on the Dynamic Support Register*.
The application should support one of the following outcomes:
- discharge from hospital
- return from an inappropriate out of area placement
- remain in the community
All applications should be supported by:
- A current comprehensive Personal Action Plan, based on a recent individual Register Review Meeting, involving the person and their family where appropriate, and other relevant stakeholders
- A costed Service Specification / Discharge Plan. ILF Scotland may be able to support HSCPs in the development of such plans if requested. The Plan should confirm specific barriers to living in the community that ILF Scotland funding could help to overcome.

What is the Dynamic Support Register (DSR)?
The Dynamic Support Register (DSR) is a register that helps HSCPs work together with other services to plan and co-ordinate support to people with learning disabilities and complex support needs. It is used to help people receive the right support in their local community and prevent or reduce unnecessary hospital stays or inappropriate out of area placements away from home.
The Coming Home Fund is for individuals in the Red category of the DSR and is for people who are in:
- hospital
- an inappropriate out of area placement
- their community but at imminent risk of a breakdown of support that could lead to hospital admission or an inappropriate out of area placement
The Fund is used to help people receive the right support in their local community and prevent or reduce unnecessary hospital stays or inappropriate out of area placements.
